Performance Application E-Form


1. Fill out the Application E-Form below and submit it to KMR Scripts. You may also send us the information via regular mail, email or phone. From that information, we will send you  a Performance Contract, which you must sign and return with complete payment. Please make extra sure this form is COMPLETELY and CORRECTLY filled out. If we have to contact you for missing information, it will lengthen the processing time of your order.

2. Whether you provide this information by mail, email, or phone– all rehearsal materials and deposits must be paid and your signed contract received by our office before we will ship any rehearsal materials, however, no royalty payments are due until seven days after you open. Sorry, we don’t accept Purchase Orders. You may pay for your Packet with Pay Pal.  Remember that each actor must have his/her own printed script (no copying allowed). 3. The usual timetable for applying, paying for, and receiving your rehearsal materials is about two and a half weeks– so please plan accordingly. It is possible to reduce the processing time to about a week via the Internet– ask for details. There will be significant additional costs if we have to rush your order processing to meet your schedule.

4. Please be certain of your choice before you order and pay the rental for a particular show. There are NO REFUNDS on rental fees for rehearsal materials once they have left our offices. Be sure to check to see if the rights to a particular show are available before you have your brochures printed up.

2.  What show do you plan to produce:  
NOTE:  You must apply for a performance license (Contract) whether you are producing a musical-- or non-musical version of our shows.  If you produce the non-musical version, you do not have to rent our Production Packet.  You do, however, need to purchase enough scripts for each of your actors.  See #8, below.

(If you plan to present two shows in one bill (two shows within a 2-hour period), you must order and pay for both production Packets and pay the refundable deposit for each show, however you only pay one royalty for each performance.  List the second show in the Additional Information field at the bottom of this form.

3.  Will your production be touring and/or in performance longer than 30 days?  (Performances over an extended period of time require monthly royalty payments and will be contracted with a Touring Rider that prescribes a payment schedule.

NOTE:  You only pay for the performances you actually present.  This number is used for the purposes of your contract and you are given permission to present more -- or less-- but you agree to pay royalty for each performance given.

11.  I understand the present royalty rate is $35 per performance and is subject to change until contracted.  I understand I must return the rented CD and Sheet Music, fill-out and return the Final Report, and pay all royalties and fees in full within 14 days of our final performance. My Failure to do this will mean that I forfeit my $50.00 deposit.  ALSO, I agree that if I fail to return the rented CD and Sheet Music within 60 days of our final performance, KMR Scripts will consider the materials to be lost and have replacement materials produced.  I agree to pay that replacement cost of $150.00-- whether I eventually return the late materials or not.  (Choosing "No" will void your application)

Shipping Information:  We need a turn-around time of 7 days (from the day we receive your payment/signed contract until the day we ship it out to you) to process your order. We use Priority Mail (by the USPS) to ship all materials, at no cost to you, which makes it a total of approximately 10 days until you receive the materials.  If you are in a rush, we can process and ship within 24 hours using an over-night service (to MOST areas) for an additional cost of $50.00. Contact us by phone or email to get the details for RUSH processing.  Of course, all fees are based on US currency and non-US shipping charges are extra.
